  • your name & phone number
  • the name & phone number of event facilitator (if different)
  • Set up requirements for tables & chairs
  • Any AV needs (video, slides, music, or microphone)
  • Anything else you would like for us to know about your request 

If there is a conflict, both facilitors will need to contact the Church Office with alternative arrangements.
